We accept Blue Cross Blue Shield Insurance only

• Patients are responsible for co-pay or co-insurance at the time of checking in.

• Patients' responsibility to understand their own insurance/reimbursement policies.

Self Pay

All other insurance or no insurance is considered self pay.

• Self pay patients are responsible for payment in full at time of service

• Self pay patients will get a super bill that they can submit to their own insurance for reimbursement.
