Insurance:
Blue Cross Blue Shield insurance accepted
*Patient is still responsible for co-pay at time of visit.
*Insurance plans differ per patient and patient is responsible for knowing their own plan and coverage including deductible, co-pay and co-insurance for both office visits and labs.
Other Insurance or No Insurance
All other insurance outside of BCBS or no insurance is considered “self pay”
*All patients will receive a Super Bill (receipt) that they can then turn into their own insurance company for direct reimbursement, if desired.
Fees:
Fees are not billed to insurance and are due at time of visit or service.

We accept Checks, Credit (Visa, Mastercard, Discover American Express), Debit, HSA, FSA
