*Insurance plans differ per patient and patient is responsible for knowing their own plan and coverage including deductible, co-pay and co-insurance for both office visits and labs.
Other Insurance or No Insurance
All other insurance outside of BCBS or no insurance is considered “self pay”
*All patients will receive a Super Bill (detailed receipt) that they can then turn into their own insurance company for direct reimbursement, if desired.
Fees:
All fees are not billed to insurance and are due at time of visit or service.
